401(k) Plan
Coblaco Services, Inc. offers a safe harbor 401(k) plan to its employees after 6 months of employment with the following matching contributions: dollar-for-dollar match on the first 3% and fifty cents on each dollar on the next 2% of deferred compensation. Employees may contribute up to $15,000 per year.
Health Insurance
To meet our employee’s health care needs, Coblaco Services, Inc. offers 2 Kaiser Permanente health care plans. One plan is a traditional HMO plan, with set co-payment amounts for medical services; the second plan is a $2,000 deductible HMO plan which also has set co-payment amounts for medical services, however, some services are subject to the $2,000 deductible.
Dental Insurance
A traditional dental plan provides levels of payment for eligible dental expenses and has no network requirements. A dental HMO provides scheduled dental services though a network of dentist. Employees absorb the cost of their chosen plan.
AFLAC
Employees have access to AFLAC provided supplemental insurance plans at their own expense.
Life Insurance
Coblaco provides a $15,000 life insurance policy at no cost to the employee.
